Meet one or more of the following criteria:
A Service Delivery Plan is designed to support each patient’s individualized treatment plan and aftercare focus. Each patient is under the supervision of a psychiatrist who is trained in the simultaneous relevance of biological, psychological and social factors. Patients also derive therapeutic benefits from working in a secure environment with staff who role model appropriate behavior. Each patient is taught to understand peer support, practice skill development and master impulsive behavior.
Based on the assessment and orders obtained from the admitting psychiatrist, a master treatment plan is initiated. This plan is formally reviewed by the attending psychiatrist and program staff on an ongoing basis. Daily review of the treatment and patient progress occurs with the physician and nursing staff.
Discharge planning is initiated at the time of admission, then formulated and revised throughout the treatment stay. A written aftercare plan is developed with participation of the patient, family member or authorized representative and the hospital staff. It includes recommendations for further treatment and/or follow-up activities. Intermountain Hospital accepts most insurance plans.
